Application Form

The student applying for membership to SALUTE agrees to the following conditions:

• Abide by all guidelines as set forth by the SALUTE National Honor Society Executive Steering Committee;
• Maintain the integrity of the honor society;
• Submit unofficial transcripts with cumulative GPA and a copy of military orders/DD214 with Honorable Discharge to the SALUTE Chapter Director for verification;
• Submit a payment of $45 lifetime membership fee upon initial application. Payment may be made by credit or debit card or by check payable to Student Veterans Association (arrange payment with the SALUTE Chapter Director).
