Information Technology Forms
Information Technology makes available to faculty, staff, and students of A&M-Texarkana many different forms for requesting access to different IT Systems. The PDF format is for viewing with Adobe Acrobat Reader DC or any other PDF reader you may have already. If you need to request installation of a PDF reader on your workstation, please submit an IT Service Desk request.
- To contact the IT Service Desk, log into Service Desk Portal with your Eagle ID.
- Alternatively, you can call 903-334-6603 or email informationtechnology@tamut.edu for submission of a ticket.
To apply for a Computer/Email account for an Adjunct, Part-Time employee, Guest or a Visitor:
A current University Supervisor or Sponsor should make the request and log into the IT Service Desk portal and:
- After logging into Service Desk Portal , choose New Request>Request A New Service>Access>Adjunct-Part-Time-Visitor Computer-Email Request
- Fill in the form and click Submit
- The request will be sent to the University supervisor or sponsor and/or Human Resources for their approval
- After receiving approvals, the account will be created
